Thank you for your interest in our programs and birthday parties! Before proceeding, we kindly ask that you reach out to us directly at (516)-469-0313 to confirm availability for your desired dates. This ensures that we can accommodate your preferred schedule and provide you with the best experience possible. Once availability is confirmed, you can proceed to register by filling out the form below. Upon registration, you'll have the option to automatically add all program dates to your calendar for your convenience. Please note: If you would like Coach Gad Azra to attend as one of the coaches, and additional fee of $150/hour will apply. Please mention this when you call to confirm and select the appropriate option from the drop down menu when completing your booking. If party size is bigger than 50 kids, please contact us in advance 516-469-0313.
Cancellation Policy
Birthday Party Policies & Liability Waiver By booking a party with Jewish Youth Sports / JYS GAD LLC, you acknowledge and agree to the following: Booking & Payment * Availability must be confirmed prior to booking. * Deposits/payments are non-refundable. * Date changes are subject to availability. Conduct * We expect respectful behavior from all children, guests, and adults attending. * Unsafe, disruptive, or inappropriate behavior may result in removal from the event without refund. Participation & Supervision * Participation in sports and physical activities involves inherent risks, including possible injury. * Parents/guardians voluntarily assume all risks related to participation. * Parents/guardians are responsible for disclosing any medical conditions, allergies, or limitations prior to the event. Liability Release * By booking, you release and hold harmless JYS GAD LLC, its owners, staff, coaches, and affiliates from liability for injuries, illness, damages, or losses arising from participation, except where prohibited by law. Communication Privacy * By providing your phone number, you consent to receive booking-related text messages and updates. * Your phone number will never be sold or shared for marketing purposes. Reply STOP to opt out. Health Acknowledgment * Guests attend voluntarily and assume responsibility for exposure to common illnesses, including communicable diseases.
